Visa Application Form: A Visa Application Form that is completely filled and signed by the Applicant. (Use only one pen to fill the form; overwriting /using whitener is not allowed)

Coloured Photograph: 2 Passport size colour Photograph 35mm x 45 mm white background and 80% face cover

Cover Letter: A covering letter from the applicant mentioning name, designation, passport number, purpose and duration of visit.. For self-employed/businessmen, the letter should be on their business Letter head for employed individuals it should be on company’s letterhead and others on the plain paper.

Travel Bookings: Confirmed air tickets or booking of air tickets of onward and return

Travel Insurance: Travel Insurance covering COVID 19 issued by Forte De Cambodia (Minimum 16000 USD)

Financial Document: Bank statement for last 3 months

Vaccination Certificate: Covid 19 Vaccination Certificate for both doses (it should be linked with Passport Number)

Other Mandatory Documents : Occupation ,Email id, and Mobile number.

Passport: scanned colour passport (First and last page) in JPEG format..Scan copy of passport with minimum six months validity

Coloured Photograph: Scanned photo should be 35x45mm, in white background and 80% face cover.
